Gang Reduction Youth Development (GRYD)

Under the supervision of the Program Director, the Case Manager is responsible for case management for program participants in the youth gang prevention program.  Will perform evaluations and develop service plans in compliance with the GRYD Prevention Model of Services.  Assist clients with goal planning and conduct regular reviews, provide support, assist in connecting clients with available community resources, and develop and maintain interagency relationships. Case Manager will provide case management services to prevention youth in collaboration with varying components of the GRYD Comprehensive Strategy.

Qualifications:  A.A. degree plus two (2) years paid experience or volunteer in a mental health OR BA or BS Degree from accredited college/university in a health related field; i.e. social work, psychology or sociology plus one (1) year experience paid or volunteer in a mental health.  Strong knowledge of case management and mental health rehabilitation principles.  Demonstrated ability to relate to culturally diverse clientele.  Must be able to work as part of a team and independently.  Ability to exercise sound judgment in crisis situation   Effective verbal and written communication skills.  Detail oriented.   Strong and timely documentation and assessment skills.  Able to work evenings and weekends when needed (hours are 10:00 am -7:00 pm.  A valid CA driver’s license, reliable transportation, proof of auto insurance liability insurance, and acceptable driving record.  Bilingual in English/Spanish required.

Salary: $27 - $29/hour
