Law Enforcement Assisted Diversion Intensive Case Management Services (LEAD-ICMS)

Provide intensive case management services consistent with the LEAD-ICMS model including but not limited to, provide in-depth, participant-centered, linguistically and culturally appropriate case management services, advocate for individuals and communities impacted by substance use, and provide harm reduction and Housing First framework through the implementation of intensive case management services. Develop and manage relationships within the community to connect LEAD participants with employment, housing, educational opportunities, vocational training, and other necessary services. Engage in high levels of collaboration and program implementation with LEAD partners, overseeing the referral process, access to care and transition plans for individuals referred to this program. Continually assess needs and provide advocacy support in obtaining appropriate services. Track regular ongoing participant contact based on acuity of needs. Maintain appropriate documentation in accordance with contract and agency requirements; preparing reports as necessary.

Qualifications:  Bachelor's Degree in social work, psychology or counseling OR experience in Case Management and System Navigation preferred. Must be familiar with crises management and able to work in the community/field. Experience, knowledge, skill and abilities with culturally diverse population preferred. Effective written and oral communication skills. Demonstrated ability to work independently and as part of the team. Ability to think independently, creatively and effectively in critical situations. Detailed oriented and time management. Bilingual English/Spanish required. Must have reliable transportation, valid CA Driver's license, must maintain current automobile insurance and an acceptable driving record. 

Salary: $24 - $27/hour
