Reentry Case Manager

The Reentry Case Manager is responsible for providing ongoing case management support to Case Managers/Credible Messengers for youth participants, including linking them to external resources for long-term care in preparation for release and reintegration with their families and communities. Will play a crucial role in the Credible Messenger model, focusing on the well-being, personal development, and community participation of youth currently incarcerated, with a special emphasis on those with developmental disabilities. Identify and engage potential clients within the juvenile justice system, particularly those with developmental disabilities, to provide direct case management services.  Monitor and support youth participation in Credible Messenger Programming and other supportive services, both pre-release and during reentry.  Maintain accurate and up-to-date case documentation.  

Qualifications:  Bachelor's Degree in Social Work, Psychology, Criminal Justice, or a related field preferred; Associate Degree with significant experience will be considered.  Minimum of two years of experience in case management, preferably with youth in the juvenile justice system or those with developmental disabilities.  Strong understanding of the challenges faced by youth with developmental disabilities and those impacted by incarceration.  Ability to work effectively as part of a multidisciplinary team and independently.  Excellent verbal and written communication skills, with the ability to engage effectively with diverse populations.  Strong organizational and documentation skills, with attention to detail.  Bilingual in English/Spanish highly preferred.  Available to work flexible hours, including evenings and weekends, to meet the needs of youth and program operations (hours at camp 11:00 am - 7:30 pm).  Valid CA driver's license, reliable transportation, and proof of auto insurance required.

Salary: $22 - $25/hour
